Information is gathered regularly to provide a source of data for the generation of an outcomes measurement report. The Executive Director or representative compiles this information annually and a management report is completed. New referrals are given surveys as they move through the various stages of our programs. Consumers placed in a DMH supported employment service are asked about satisfaction during monthly reviews and sent satisfaction surveys annually. Consumer Panel meetings are held quarterly. All referral/funding sources and employers are surveyed annually as well.
